To schedule an appointment please call the NYCAMH office during the accepting appointments date range listed below for the clinic you would like to attend, and ask to speak with the farm respirator clinic scheduler.
Call 607-547-6023 or (toll free) 800-343-7527 - Have the following information ready when calling
1. Total number of people attending from your farm
2. Name of each person being scheduled
3. Language spoken by each attendee
4. Make & model of each respirator to be tested. - if a worker uses more than one style of respirator they must be fit test for each one.
